ORA-00600[kql-hash-collision] Errors Reported in Alert Log (Doc ID 752115.1)

Oracle Server - Enterprise Edition - Version: 10.2.0.4
This problem can occur on any platform.

Symptoms

The following symptoms have been attributed to this issue :-

a) The following error or similar is seen in the alert log :-

ORA-00600: internal error code, arguments: [kql-hash-collision], [SYS], [.], [DBMS_STANDARD]

The arguments will vary but first argument will be 

b) Current SQL statement for this session is very generic, the identified bug relates
to concurrent DDLs running on the system and DDL triggers being present

c) The call stack trace will be similar to :-

ksedst <- ksedmp <- ksfdmp <- kgerinv <- kgeasnmierr
kqllod kglobld kglobpn kglpim kglpin kglgob kgiinb
pfri7_inst_body_common <- pfri3_inst_body <- pfrrun
plsql_run <- peicnt

The key function for this article to be considered to be 'kqllod'
