Streams Apply Fails With ORA-7445 [kgscDump] and ORA-7445 [kxsxsi]

Streams Apply Reader process may abort due to ORA-7445 errors:
In the alert log you may see errors similar to:
ORA-07445: exception encountered: core dump [kxsxsi()+414] [SIGSEGV] [Address not mapped to object] [0x000100009]  
Streams Apply Reader P000 pid=... OS id=... stopped
ORA-10388: parallel query server interrupt (failure)
In the trace file you may see a Streams update with many bind variables:
insert into "STREAMADMIN"."QUEUE_SCHEMA_TRS" (q_name, msgid, corrid, priority, state, delay, expiration, time_manager_info, local_order_no, chain_no, enq_time, step_no, enq_uid, enq_tid, retry_count, exception_qschema, exception_queue, recipient_key, dequeue_msgid, user_data, sender_name, sender_address, sender_protocol, user_prop, cscn, dscn) values (:1, :2, :3, :4, :5, :6, :7, :8, :9, :10, :11, :12, :13, :14, 0, :15, :16, :17, :18, :19, :20, :21, :22, :23, :24, :25)

Searching through the trace file further, you may also see indications of ORA-01438.
