Symptoms

Database went down due to ORA-600 errors in PMON

PSP0: terminating instance due to error 472

The likely sequence of the ora-600 [ktspfupdst-1] will be:
- raised in user background process
- raised in pmon which lead to an instance crash (cleanup of previously crashed process)
- raised in smon which again lead to instance crash
