RAC: SRVCTL Cannot Start Database or Instances - ORA-01565 Encountered in imon log

Symptom 1. The database and instances cannot be started with srvctl.

For example:

$ srvctl start instance -d PROD -i PROD1
PRKP-1001 : Error starting instance PROD1 on node oradb1
CRS-0215: Could not start resource 'ora.PROD.PROD1.inst'.

The reason can be determined by checking the resource log in the $RDBMS_HOME/log/[nodename]/racg directory.  The file is named imon_[dbname].

Symptom 2. The imon log shows ORA-01565.

For example:

SQL> ORA-01565: error in identifying file '+DATA_DG/PROD/spfilePROD.ora'
2009-03-05 02:07:33.519: [ RACG][6] [6031][6][ora.PROD.PROD1.inst]: -17503: ksfdopn:2 Failed
to open file +DATA_DG/PROD/spfilePROD.ora
ORA-15056: additional error message
ORA-17503: ksfdopn:DGOpenFile05 Failed to open file +DATA_DG/PROD/spfilePRO
ORA-01078: failure in processing system parameters
SQL> Disconnected


Symptom 3. Starting the database manually with sqlplus may also fail with the same error.

For example:

SQL*Plus: Release - Production on Mon Mar 16 00:05:24 2009

Connected to an idle instance.

SQL> startup
ORA-01565: error in identifying file '+DATA_DG/PROD/spfilePROD.ora'
ORA-17503: ksfdopn:2 Failed to open file +DATA_DG/PROD/spfilePROD.ora
ORA-01000: maximum open cursors exceeded
SQL> exit
