ORA-600 [kcbw_get_bh_1] and/or ORA-7445 [kcbw_dump_granule()+775] errors are raised.
This can also show up as ORA-7445 [kcbw_get_le] and ORA-7445 [kcbw_get_sim] errors.

The stack dump will be similar to:

In either case, the call stack will show module kcbw_dump_quiesce_list called from ksmg_create_request.
