ORA-00600 [qmx: no ref] Xdb Uninitialized Xdb$Schema Not Accessible (Doc ID 944088.1)

Symptoms

While installing XDB, the following errors are encountered

Errors in file /obase/diag/rdbms/<sid>/<sid>/trace/<sid>_ora_950348.trc  (incident=36368):
ORA-00600: internal error code, arguments: [qmx: no ref], [], [], [], [], [], [], [], [], [], [], []
Errors in file /obase/diag/rdbms/<sid>/<sid>/trace/<sid>_ora_950348.trc:
ORA-31159: XML DB is in an invalid state

Or a database that was working fine is suddenly generating the following error in the alert.log:

Errors in file /v40/app/oracle/diag/rdbms/<sid>/<sid>/trace/<sid>_ora_9391.trc  (incident=121297):
ORA-00600: internal error code, arguments: [qmx: no ref], [], [], [], [], [], [], [], [], [], [], []

The trace file shows that the failing sql (Current SQL statement) involves XDB, examples
