CSS-00168: failed to report individual exceptions; CSS-00122: failed to create scan report

Encountered the following issue when running CSSCAN:
CSS-00122: failed to create scan report
Further diagnosis shows:
No space issues in the SYS tablespace or in the CSM$ERRORS table
When CAPTURE=Y was used in the CSSCAN command and SUPPRESS=<number> was not used, issue still occurred
When CAPTURE=Y was used in the CSSCAN command and SUPPRESS=<number> was used, issue did not occur; however, results in the CSSCAN were not complete and this was not a viable option as the CSSCAN would have to be run too many times to capture and fix all of the errors.
The test database was continuously refreshed from the production database.
