Instance Crashed After ORA-07445 [ksrconsume]

LGWR encounters the following error and as a result the instance crashed:

ORA-07445: exception encountered: core dump [ksrconsume()+665] [SIGFPE] [ADDR:0xA561D3] [PC:0xA561D3] [Integer divide by zero] []

As LGWR crashed, FSF could not fail over the instance.

Problem is not specific to LGWR and it may affect also others processes.

The trace file of the process that hit the ORA-7445 [ksrconsume] shows a 'broadcast handle' state object where the event count has rapped ("messages published: 0"), for example:
