Extra NESTED LOOPS Step In Explain Plan on 11g and Above (Doc ID 978496.1)

Symptoms

When looking at the access path for a query in 11g, the plan looks different when compared to 10g.
The 11g plan contains an extra nested loops step.
The row source plan is the same as the explain plan.
