Workflow Diagram / Status Monitor Not Loading

Symptoms
Navigating to Status Monitor Page, then select any workflow item type & item key , click on Action History and expecting to see the Workflow Diagram. Unfortunately, the Applet is not initializing when trying to launch the Workflow Diagram. It gets stuck and finally is going to timeout.
