iSupport: Duplicate Attachments Appear when Creating Service Requests (Doc ID 1060750.1)

When creating a Service Request through iSupport and attaching a single document, duplicated attachments appear even though only one file was attached.

Expected Behavior:
Expected to see only 1 copy of each attachment

Steps To Reproduce:
The issue can be reproduced at will with the following steps:
1. Create a Service Request using an iSupport responsibility.
2. Add an attachment to the Service Request
3. Query the Service Request and 2 attachments appear even though a single process has been performed
