Repair Order Error: 'ORA-01422: exact fetch returns more than requested number of rows ' When Creating A Contact

Symptoms
Can not create a Contact from the Repair Order form.
The contact cannot be saved
OR get the error:
ORA-01422: exact fetch returns more than requested number of rows
The issue can be reproduced at will with the following steps:
1. Open the Repair Order s form.
2. Select 'New' or 'Open'
3. Select a Customer, and then cick the 'Create Contact' button.
4. Enter contact details (for 'Employee Of') and save.
Note the reported error.
This is only occurring for the 'Employee Of' type.
