Symptoms

When creating Receipts using iProcurement for Standard Purchase Orders the following error occurs

 

Step to reproduce:

1. Navigation: Purchasing Responsibility->Receiving -> Receipts
2. Enter Receipts Form
Enter Receipts information and save.
( On attempting to save, get an error message):  
   
        APP-204 "Concurrent Manager encountered an error while the spawned
        concurrent program" Receiving Transaction Manager RCVOLTM for your
        concurrent request 164463. TM-SVC LOCK handle failed.

Additional setup information:

Profile: RCV: Processing Mode - ONLINE


Also, a different error may be encountered in later releases:
        CONC-TM-NO MANAGER DEFINED

The issue also occurs entering receipts in iProcurement
