Service Charges Report: An error encountered either due to invalid Template details or due to null Data Input Stream (Doc ID 1068524.1)

Symptoms

Attempting to run the Charges Report, the following error occurs:

Error:
An error encountered either due to invalid Template details or due to null Data Input Stream.


STEPS
-----------------------
The issue can be reproduced at will with the following steps:

1. From a responsibility leading to the Service request Form > Find SR (enter SR number)

2. Charges tab> (B) Charges Report
The jsp page launches: ... /OA_HTML/RF.jsp

3. Select Template Service Request Details
Select Language e.g. Locale English: United States
Select format: e.g  HTML
(B) Run


Due to this issue, users cannot produce report on charges.

Log files show the following errors or more:
