The workflow fails with Document Manager Error number 3 at the Verify Authority activity after one user approves requisitions.

      Failed activity: Does Approver Have Authority? (VERIFY_APPROVER_AUTHORITY)
      Error Stack: Wf_Engine_Util.Function_Call(PO_REQAPPROVAL_ACTION.VERIFY_AUTHORITY,
      POAPPRV, 133890-113857, 196532, RUN)
      Error Message: User-Defined Exception

sysadmin error:
      po.plsql.DOCUMENT_ACTION_CHECK.authority_checks_setup:40:100 ORA-01403: no data found
      po.plsql.PO_DOCUMENT_ACTION_CHECK.authority_check:20:unexpected error in authority_checks_setup
      po.plsql.PO_DOCUMENT_ACTION_PVT.do_action:110:unexpected error in action call

OR

Steps:
1. Enter requisition
2. Submit it for approval
3. Log in as the approver and respond to the notification
4. Workflow fails with document manager error and requisition is stuck with an in-process status
