WF_DEFERRED_R is Invalid After Upgrade From 11.5.10 to 12.0.1 (12.1, 12.2), or Database Upgrade to 12c, or Table Re-organization

Symptoms
After upgrading from Oracle Applications 11.5.10.2 to Release 12.0.1, the Workflow "WF_DEFERRED_R" Rule Set is Invalid and cannot be validated.
* These steps can be applicable beyond Release 12.0.1, because the problem is most commonly caused by Database changes. ex: upgrade to 12c, or table re-organization
This can happen to any one or all of the Workflow Queue Rules:
