Symptoms

When clicking on Launch button in Activity Workbench form,  the following error occurs.

ERROR

ORA-01422: exact fetch returns more than requested number of rows
ORA-06512 EAM_ACTIVITYUTILITIES_PVT


STEPS

The issue can be reproduced at will with the following steps:
1. Navigation: Activities -> Activity Workbench.
2. Click on Launch button and receive the above error.
