Symptoms
You are using the Microsoft Internet Explorer 8 (IE8) or later (through IE11) to access multiple Release 12.1.x E-Business-Suite Instances
When you log out from one instance and login to the second or third instance, the following error is received:

Unable to authenticate session
...
For one Instance logon is working, but for other Instances (in most cases a clone) it is not working and the above error occurs.
