R12: "FRM-92101: There Was a Failure in the Forms Server During Startup" Error When Executing EXECUTE_QUERY in Forms (Doc ID 1111113.1)

Symptoms

On Ebusiness suite Release 12.1.3, Forms issues

Forms runtime process crashes when executing EXECUTE_QUERY the following error occurs:

FRM-92101: There was a failure in the Forms Server during startup.
This could happen due to invalid configuration

The application.log file shows:

10/05/17 11:02:40.263 formsweb: Forms session <10> aborted: unable to communicate with runtime process.
10/05/17 11:02:40.264 formsweb: Forms session <10> exception stack trace:
java.io.IOException: FRM-93000: Unexpected internal error.
Details : No HTTP headers received from runform
