OAFM OC4J Stops Responding Sporadically Resulting in SOA Connection Leaks (Doc ID 1121499.1)

Symptoms

In a 12.1.1 E-Business Suite/SOA Gateway environment, the oafm oc4j sporadically stops responding and causing transactions to no longer be processed.

The OAFM Server logs show the following:
