Symptoms

On : 11.5.10.0 version, Service Request Form

Find that in the Service Request form, for certain customer and Installed Base instance combination, the "Incident Customer" field is empty.

EXPECTED BEHAVIOR
-----------------------
Expect the Incident Customer field to have a value.

STEPS
-----------------------
The issue can be reproduced at will with the following steps:
1. In Service Request form query the affected SR.
2. The Incident Customer field is empty.
