Message "The System Could Not Determine The Internal Source For Item" When Adding Item To Cart

On R12, when attempting to add an item to cart the following error occurs.
"The system could not determine the internal source for item 12345. Contact purchasing administrator"
Internal Requisitions should not be used and Item should be purchased externally - from vendor.
Steps to Reproduce:
1. Create a new item in Master Items form.
2. Add to cart.
3. Get error message.
