Error 'OracleSSWA.Execute Was Not Found On This Server' When Using Preferences SSWA / Change Session Language (Doc ID 1185893.1)

Symptoms

After upgrading from 11i to R12:
Errors occur under Preferences SSWA when choosing either "Change Session Language" or "General Preferences". 
Instead of the OAF web page showing options for session language, a web page with the following error appears:

Error: The requested URL /pls/DEV10g/OracleSSWA.Execute was not found on this server

 
The issue can be reproduced at will with the following steps:
1. connect SYSADMIN (or any other user with Preferences SSWA responsibility)
2. choose "Preferences SSWA" responsibility
3. choose "Change Session Language" or choose "General Preferences" in menu

Changes

Upgrade from 11i to R12
or a fresh install of R12 VISION environment
