Approval Workflow Database Error Occurs While Approving Purchase Requisition (Doc ID 1191233.1)

Whenever a requisition is submitted for approval by changing the Managerial approver and inserting another viewer, the Req is stuck in INCOMPLETE Status.

Issue reproducible and started after upgrading to 12.1.1 from 12.0.4

Expect Requisition not to Stuck in INCOMPLETE status

Steps to Reproduce:
The issue can be reproduced at will with the following steps:

     1. Create a Req.

     2. Change Managerial Approver and also insert a viewer.

     3. Submit for approval.
