After R12 upgrade child and parent relationship of DFF incorrect (Doc ID 1213463.1)

Symptoms

After the upgrade from 11.5.10 to Release 12.1.2, the fnd flexfield hierachies are not correct. In some cases, parent-child relationships that were deleted in 11.5, now exist in R12. For example: Parent R349 - Child 2772, Parent R518 - Child 2772. These relationships do not exist in 11.5, as they were deleted months ago, but they do exist in the R12 environments.

It is expected that fnd flex values and hierarchies are migrated exactly as they are in 11.5.

The issue can be reproduced at will with the following steps:
1. Upgrade from 11.5.10.2 to 12.1.2
2. Review the values and relationships in the Location segment of the accounting flexfield
