Form Using FlexField crashes with FRM-93000: Unexpected Internal Error in application.log (Doc ID 1238633.1)

Symptoms

Users receiving an FRM-92101 error in the Forms applet indicating the connection to the associated Forms Runtime (frmweb) process on the server was lost using a Form using FlexField(s), for example the following navigation:

Receivable Manager > Setup > Financials > Accounting >Ledger > Accounting Setups
Query either existing ledger
Click "Update Accounting Options" icon
Click "Update" icon from "Operating Units"
From operating units screen, select "Open Organization Form" button
Query OU with "Find Organization" screen
Select organization classification "GRE/Legal Entity" and click "Others" button

The application.log of the Forms OC4J ($LOG_HOME/ora/10.1.3/j2ee/forms/forms_default_group_<x>  {<x>=1,2,3,...} reports the following

<timesstamp> formsweb: Forms session <7> aborted: unable to communicate with runtime process.
<timesstamp> formsweb: Forms session <7> exception stack trace:
java.io.IOException: FRM-93000: Unexpected internal error.
