TeleService Errors After Upgrade: FRM-41045 in Contact Center, FRM-40735 or FRM-47023 in Service Request

Symptoms
After upgrading or applying TeleService patches, users may encounter errors when trying to open the Contact Center form or trying to query an existing Service Request in the Create Service Request form.
Errors:
* In the Contact Center:
FRM-41045: Cannot find item: invalid ID.
FRM-41045: Cannot find item: invalid ID.
FRM-47023: No such parameter named DB_SITE_ID exists in form CSCCCCRC
FRM-40105: Unable to resolve reference to item PARAMETER.DB_SITE_ID.
FRM-40105: Unable to resolve reference to item DB_CONTROL.VIEW_BY_SELECTION.
FRM-40735 the trigger WHEN-NEW-BLOCK-INSTANCE raised an unhandled exception ORA-06508
followed by
FRM-40735 the trigger CLOSE_WINDOW raised an unhandled exception ORA-06508
* In the Service Request form:
FRM-40735: CLOSE_WINDOW trigger raised unhandled exception ORA-06508.
-or-
FRM-40735: POST-FORM trigger raised unhandled exception ORA-06508
* Accessing the Service Request form:
FRM-47023: No such parameter named SPECIAL_MENU_PARAMETER42 exists in form CSXSRISR.
FRM-40105: Unable to resolve reference to item PARAMETER.SPECIAL_MENU_PARAMETER42.
FRM-41045: Cannot find item: Invalid ID.
or
FRM-47023: No such parameter named SPECIAL_MENU_PARAMETER42 exists in form CSXSRISR.
FRM-40105: Unable to resolve reference to item PARAMETER.SPECIAL_MENU_PARAMETER42.
FRM-40738: Argument 3 to builtin SET_MENU_ITEM_PROPERTY cannot be null.

FRM-41084: Error getting Group Cell.

(N) Customer Management > Contact Center
(R) Service
(N) Service Request > Create Service Request
Changes
- Upgrade from 11.5.10 or Release 12 to a higher version.
- Apply Service patches.
