Customer Form, Contacts Issue: After R12 Upgrade, Contact & Contact Roles In Table Do Not Appear On Communication Tab (Doc ID 1264148.1)

Symptoms

After upgrading from 11.5.8 to R12.0.6, Contact Roles that exist in the tables do not appear on the Communication tab. The issue seems to happen only for contacts that were created via Oracle iStore before the upgrade. 
Contacts that were added manually through Oracle Receivables Customer Form before and after the upgrade do not have any issues.
