While trying to attach a file for an Item, problems occur with using the item document attachment feature in the Items form.  Clicking on the item attachment link the upload file page does not occur instead login page occurs and after entering the credentials, it errors with the message 'insufficient privileges for the operation'.

Note: This is happening throughout the R12.06 instance and for all the users.
