Unable to Create New Code Combination. Asks for Confirmation when Closing the Form. ORA-01407 (Doc ID 1278517.1)

Symptoms

While creating new code combination, the changes are not being saved. Even after clicking on save, when the Form is closed, it is again asking for a Save.

Trace file is showing ORA-01407 (err=1407) for the following statement:
UPDATE gl_code_combinations cc
SET account_type = (SELECT decode(vs.flex_value, 'T', 'O', substrb( fnd_global.newline||vs.compiled_value_attributes||fnd_global.newline,
instrb( fnd_global.newline||vs.compiled_value_attributes||fnd_global.newline,

FROM fnd_flex_values vs
WHERE vs.flex_value_set_id = 1006845 AND vs.flex_value = cc.SEGMENT4 ),

EXPECTED BEHAVIOR

Expect new code combinations to get saved on saving.

STEPS
-----------------------
The issue can be reproduced at will with the following steps:
1. General Ledger > Setup > Account > Combinations
2. Put the new combination and press on Save Bottom
3. Close the form and the message appears " Do you want to save ''
