Geography Hierarchy (Generic Datafix Provided): States, Counties, Cities in 11i Have Not Migrated into Release 12 Geography Hierarchy Table

Symptoms
After migrating from release 11i to R12, not all State, County, City values were migrated from the table AR_LOCATION_VALUES to the table HZ_GEOGRAPHIES.
The values are not displaying on the Geography Hierarchy form.
This issue is happening when the 11i Location Flexfield Structure as COUNTRY.STATE.CITY.
The sales tax location flexfield was set to COUNTRY, STATE,CITY.
