Symptoms


When some users try to open the Create Service Requests form, they get an error.
Other users are successful.

Individual Logon IDs are consistently successful or not successful.

Errors:

FRM-41045: Cannot find item: invalid ID

or

FRM-41017- Cannot set UPDATE ALLOWED attribute of non-enable item INCIDENT_TRACKING.INCIDENT_CITY
FRM-41017- Cannot set UPDATE ALLOWED attribute of non-enable item INCIDENT_TRACKING.INCIDENT_STATE


Steps:

(R) Customer Support
(N) Service Requests > Create Service Requests

What is Working:

Some logon ids (using SAME responsibility) are successful in accessing the Form.

Changes

Upgraded instance from 11i or 12.0.4 to 12.1.3.
(Regardless of RDBMS upgrade)
