STEPS

The issue can be reproduced at will with the following steps:
Order Weight Modifier not working.
Modifier Type: Misc/Freight Charge-US Modifier Type: Freight and Special charge Currency: USD
Charge Name: FREIGHT/MISC CHARGES
Applictaion method: Lumpsum
Value: 25

Line-Level Qualifier:

Grouping Number Qualifier Context Qualifier Attribute Precedence Operator Value From Value To
-1 Customer Customer Name
-1 Volume Order Weight 640 BETWEEN 0 59.99
-1 Order Order Type 470 = STANDARD


After setting this modifier we did the following:
1) Profile QP: Line Weight UOM Code is set as LBS in Site Level
2) Run "Build Attribute Mapping Rules "
3) Run "QP: Maintains the denormalized data in QP Qualifiers"

Order  is entered into system has no freight charges although it satisfies all qualifiers for the Modifier.
