In a Notification, the Note Value Entered By One Approver Is Not Getting Saved While Forwarding A Purchase Order to the Next Approver (Doc ID 1300493.1)

Symptoms

On 11.5.10.2 version with Procurement Rollup Patch 12 applied Plus Patch 10273050, find that when an approver enters a value in the Note field in the approval notification for a Purchase Order and sends to the next approver, the text from the previous approver in the Note column of the action history is not there. 
Expect that next approver to see the data entered from the previous approver in the Action History / Note column in the notification

Steps to Reproduce:

1. Send Purchase Order for approval to an approver
2. Approver opens this notification and enters data in the Note field and sends to next approver
3. Next approver opens the notification, but cannot see the date entered from the previous approver in the Note column
