Symptoms
When trying to access EBS R12 login you will get "404 Not Found" for both AppsLogin and AppsLocalLogin and checking on the Apache error_log you see message like
File does not exist: /apps/oracle/PROD/inst/apps/PROD/portal/OA_HTML/AppsLocalLogin.jsp
or
File does not exist: /apps/oracle/PROD/inst/apps/PROD/portal/OA_HTML/AppsLogin.jsp
