11i Contact Center Error: FRM-47023: No such parameter - Unable to Open Form Listed in Quick Menu (Doc ID 1314998.1)

Symptoms

Set up a Quick Menu and add the particular form (function) to the menu. Once this has been defined navigate to the form you you want to access the Quick Menu from. Bring up the Quick Menu and select the new form you want to bring up. Errors similar to below appear:

FRM-47023: No such parameter named CS_QM_FROM_QM exists in form OEXOEORD.
FRM-47023: No such parameter named CS_QM_FUNCTION_ID exists in form OEXOEORD.
FRM-40105: Unable to resolve reference to item PARMETER.G_QUERY_FIND
