Symptoms
On : 12.1.1 version, Transaction Entry:
After upgrade from 12.1.1 to 12.1.3, when attempting to open Order Organizer, the following error occurs.

FRM-40735: PRE-FORM trigger raised unhandled exception ORA-06502.
and
ORA-01403: no data found
