MS Outlook Approve Link Does Not Respond For Purchase Order Approval Notification

Symptoms

Using MS Outlook, on email notifications (that require approval by the approver) when choosing the Approve link, there is no response. When the user clicks the link, instead of a new response email with the generic response template opening, nothing happens.  The Note field has a long value when the issue occurs.

STEPS
The issue can be reproduced at will with the following steps:
1. User submits a purchasing document for approval with a large value in the Approval window 'Note' field.
2. Approver receives the email notification to approve.
3. Approver opens the approval notification via MS Outlook.
4. Approver clicks on Approve link and nothing happens.
