Applying Patch 12362963 Fails With "PL/SQL: ORA-00904: "Prorate_charge": Invalid Identifier" and ORA-02289: sequence does not exist (Doc ID 1338167.1)

Symptoms

On application version 12.1.1 in Development (FHRDEV) instance, applying <Patch 12362963>:R12.AP.B fails with the following error:

Error message 1
prorate_charge = NVL(prorate_charge, 'N');

ERROR at line 33:
ORA-06550: line 33, column 33:
PL/SQL: ORA-00904: "PRORATE_CHARGE": invalid identifier

When this is resolved, the following error message is received:
