Valid Ship Methods Not Displaying In LOV

ACTUAL BEHAVIOR
---------------
When a user goes to add a Shipping Method to the Sales Order, the Shipping Method is not found in the List of Values (LOV). For some reason, 8 Shipping Methods have disappeared from the list. These Shipping Methods have not been updated, nor have they been disabled. They are all active & enabled in Organizations.
EXPECTED BEHAVIOR

Expect to see all 8 of these Shipping Methods in the LOV.
STEPS
-----------------------
The issue can be reproduced at will with the following steps:
1. Carriers form with Freight Carrier Active check box checked, Ship Method: XXXX
2. Organization Assignments- Carrier Service form, which shows that XXXX has Assignments, ie. Code: yyyy, Name: yyyy_name, Assigned check box is checked.
3. Navigate: Orders, Returns, Sales Orders, Order Information tab, Others tab, Warehouse field, BED is not in the Shipping Methods LOV.
