Different Delivery Created for Every Sales Order Line on Large Orders

Symptoms
Application Release: 12.1.3
When pick releasing large sales orders each line on the order is assigned to it's own delivery.
The system is not honoring the Delivery Grouping Attributes to create a delivery 'Within Order'. All lines share the same ship from, customer and ship method. Since they share these attributes, they should be on the same delivery but are not.
The Delivery Grouping Attributes have 4 items enabled:
Ship From Location
Ship To Location
Customer
Ship Method
The expectation is that when the Delivery Grouping Attributes all match the order lines should have the same delivery assigned to them. This worked correctly in 11. 5.10.
STEPS
------
Responsibility: OM Super User
Enter (or import) the order.
Book the order.
Run pick Release.
View the delivery lines.
BUSINESS IMPACT
---------------
Due to this issue, system does not group lines together pertaining to the same ship to on the same order.
