After migrating from 11i to 12.1.3 the users reported the following errors when creating a new code combination.
Error 957 in FDFICD
APP-FND-710: An unexpected error has occurred. Please inform your system administrator that the error: ORA-957: duplicate column name occurred when the FND_FLEX_SERVER package tried to execute the SQL statement below.
APP-FND-00710: An unexpected error has occurred. Please inform your system administrator that the error: ORA-00957: duplicate column name occurred when the FND_FLEX_SERVER package tried to execute the SQL statement below.
If tables or sequences do not exist this may indicate a problem with the installation. Make sure grants and synonyms exist so that all tables, views, and sequences in the SQL statement are visible from the database account in which the FND_FLEX_SERVER package resides (usually APPLSYS).
If this is a select statement and the last portion of the where clause is in incorrect, then an invalid where clause may have been supplied in the flexfield definition. The error is probably either in the form, or in a server-side package that calls the flexfield validation routine.

insert into GL_CODE_COMBINATIONS (CODE_COMBINATION_ID, CHART_OF_ACCOUNTS_ID, ENABLED_FLAG, SUMMARY_FLAG, START_DATE_ACTIVE, END_DATE_ACTIVE, LAST_UPDATE_DATE, LAST_UPDATED_BY, DETAIL_BUDGETING_ALLOWED_FLAG, DETAIL_POSTING_ALLOWED_FLAG, REFERENCE3, ACCOUNT_TYPE, REFERENCE3, JGZZ_RECON_FLAG, SEGMENT1, SEGMENT2, SEGMENT3, SEGMENT4, SEGMENT5, SEGMENT6, SEGMENT7) values (:FND_BIND1,:FND_BIND2,:FND_BIND3,:FND_BIND4,:FND_BIND5,:FND_BIND6,sysdate,:FND_BIND7,:FND_BIND8,:FND_BIND9,:FND_BIND10,:FND_BIND11,:FND_BIND12,:FND_BIND13,:FND_BIND14,:FND_BIND15,:FND_BIND16,:FND_BIND17,:FND_BIND18,:FND_BIND19,:FND_BIND20)
the following error occurs.
These errors can be found when either creating a new account code combination dynamically or manually.
Note: A duplication was identified in the code combination's segment qualifier (id_flex_code=GL# and value attribute_type = GL_CONTROL_ACCOUNT and Control Account)
The two values use field reference3.
