Viewing Customer Address in Brazil errors with 'Value BR for the Flexfield Segment Country does not Exist in the Value Set AR_BR_LOC_COUNTRY'

After upgrading from 11.5.10 to R12.1.3, the following error is received when viewing customer address in Brazil:

"Value BR for the flexfield segment Country does not exist in the value set AR_BR_LOC_COUNTRY"

Steps to reproduce:
-Responsibilitly: AR Receivables
-Navigation: Customers . Customers
-Search for Customer “Colgate”
-Click on Customer Name
-Select the customer “Brazil” address and click on Details button
-The error appears.

South America Address Style from the Address DFF is used.

After the R12 upgrade, the context South America Address Style for Address DFF have the following segments (2 extra segments that Address DFF in 11i does not have):

2 Address Line 2 ADDRESS4
4 Address Line3 ADDRESS2
5 Province PROVINCE
5 Country COUNTRY
6 State STATE
7 City CITY
7 State Code COUNTY
8 Postal Code POSTAL_CODE

There are two duplicate segment numbers: 5 and 7. This is causing the issue.
