R12 E-Business Suite Forms Sessions are Failing with error "FRM-92101 - There was a failure in the forms server during startup" (Doc ID 1397778.1)

When attempting to use any forms based application, the forms sessions either fail to launch with the following error


FRM-92101 There was a failure in the forms server during startup

Java Exception Cannot run program "$10.1.3_ORACLE_HOME/bin/frmweb" (in directory "$10.1.3_ORACLE_HOME/forms"): error=2, No such file or directory

NOTE:- FRM-92101 error can also be reported during a forms session.

The OC4J forms_default_group_? application.log also shows

11/10/10 09:57:34.151 formsweb: Forms session <13> aborted: runtime process failed during startup with errors
Unable to switch to Working Directory: $10.1.3_ORACLE_HOME/forms

Steps to Reproduce

The issue can be reproduced at will with the following steps:

1. Log into the E-Business suite
2. Choose any forms based application
3. Failure occurs

Note: The issue can be intermittent and it is possible to launch forms if the  forms link from the homepage is re-tried.

Business Impact

Due to this issue, users are intermittently not able to connect to forms applications.
