Launching eBusiness Center Dashboard Tab Errors with FRM-92101: There was failure in the Forms Server during startup (Doc ID 1400581.1)

Symptoms

On a fresh 12.1.3 installation on a single node, Forms for App Technology

When attempting to open the Dashboard Tab in the eBusiness Center form the following error occurs.

ERROR
-----------------------
FRM-92101: There was failure in the Forms Server during startup. This could happen due to invalid configuration.  Please look into the web-server log file for details.

Details...
Java Exception:
oracle.forms.net.ConnectionException: Forms session <2> aborted: unable to communicate with runtime process.

STEPS
-----------------------
The issue can be reproduced at will with the following steps:

1:- Log into the E-Business suite
2:- Open the eBusiness Center form
3:- Click the DASHBOARD tab.

BUSINESS IMPACT
-----------------------
Due to this issue, users cannot use the Dashboard in the eBusiness Center form
