TeleService Issue With Relationship Plans After Applying Certain Consolidated Patches (Nov,Dec,Jan) (Doc ID 1416664.1)

Symptoms

Actual Behavior

While querying or saving Service Requests we are getting empty popup box.
This is happening for all the customers and all the service requests.

Expected Behavior

Relationship Plan window should not be displayed if not setup or when condition is not met.

Steps to Reproduce

(R) Field Service Dispatcher
(N) Find Service Request > Query any existing SR > Get the popup

Changes

Applied on the following Monthly Consolidated patches for 12.1.3:
- <Patch 13063024> - 12.1.3:BUG FIXES FOR CS: NOV'11
- <Patch 13337716> - 12.1.3:BUG FIXES FOR CS: DEC'11
- <Patch 13499409> - 12.1.3:BUG FIXES FOR CS: JAN'12
