Empty Customer Relationship Actions Form Pop-Up In Service Request Form After Patch 13063024 (Doc ID 1417313.1)

Symptoms

On : 12.1.3 version, Setup flow

Actual Behavior

Empty "Customer Relationship Actions" form pop-up when retrieve SR record in "Service Request" form After Patch 13063024


Steps to Reproduce

(1) Open "Service Request" form and create a new Service Request Record
(2) Under "Workbench" tab, add 2 new notes with type = 'work notes' and visibility = 'publish'
(3) Press save and 2 notes records under 'Workbench' > 'notes'
(4) When click the note description, an empty "Customer Relationship Actions" pop-up window is prompt.
(5) This pop-up window re-appear if I click another note record.
