Dynamic BI Publisher Images are not Displayed in Various Output Formats

Symptoms
Dynamic images (based off of a URL) are not appearing or displayed in all output formats.
Following the guidelines (using Microsoft Office 2003) of:
1.) Add a placeholder image.
2.) Right-click it, then on the "Size..." menu item.
3.) Click on the "Alt Text" tab.
4.) Enter the following code:
For Example:
url:{concat('http://<server>/gsm/popups/docstream.aspx?id=',ATTACHID)}
The images display in the following formats:
- HTML
- Excel
- Excel2000
The images do NOT display in the following formats :
- PDF
- RTF
- Excel2007
- PowerPoint
- PowerPoint 2007
