OAM 10g: HTTP-404 Not Found Error For EBS Login Integrated With OAM 10g

Oracle E-Business Suite (EBS) has been integrated with Oracle Access Manager (OAM) 10g, with reference to <Document 975182.1> Integrating Oracle E-Business Suite with Oracle Access Manager 10g using Oracle E-Business Suite AccessGate.
The integration is not working - after submitting credentials in the EBS login page HTTP-404 Not Found error is displayed.
Steps to reproduce
1. Access EBS http://EBSAccessGateHost.Domain:Port/ebsauth_test
2. The EBS AccessGate login page is displayed (OAMLogin.jsp).
3. Enter valid EBS / OAM username and password and Submit.
4. Error HTTP-404 Not Found is displayed.
