On 12.1.3,

When entering results in Oracle Quality, the following error occurs

APP-QA-16254: Error occurred while generating sequence number. Please contact your system administrator.


Steps to reproduce the issue
1. Quality -> Enter Results form
2. Enter results and obseve error
